The following forms must be used and completed by the panel during the selection process for redundancy or selection via the redeployment process:

1.  Chairperson's Record Sheet

This sheet must be completed to short-list each applicant from the redeployment register in line with the University's redeployment process.  It should also be completed for each individual in a selection for redundancy desk-top exercise.
The panel will also complete the Chairperson's Record Sheet together for all interview candidates being interviewed for a role as part of the University's redeployment or redundancy processes.

In both situations this sheet will enable the panel to provide in-depth feedback to all candidates and score the candidates against each of the essential criteria of the role 0-3.

2.  Scoring Matrix

Employees have the right to request feedback following the selection for redundancy and redeployment processes.  The scoring matrix should be completed as well as the Chairperson's record sheet for both short-listing and interview exercises in these instances. The scoring matrix will detail an individual's own score, along with the anonymised scores of any other candidates.

If there are only two individuals and therefore the other individual is identifiable, the consent of the other individual will be requested as the information is of a personal nature.  Where an individual refuses to give consent for disclosure of their scores, the University will take a view regarding the 'balance of interest'.
